Custody for whom?
My wife and I are still married, however, our marriage is on the rocks and she is planning to relocate to the other side of the country. we have a 2 year old son and she is just planning on taking him. What are my rights and is there any action I can take to keep my son with me?

You should immediately file for divorce, and with it file a motion for temporary emergency residential parenting of the child. Once she leaves the state, it gets a lot harder to enforce.
