Legal Question in Family Law in Ohio

My daughter is 2 months old today. Her father did sign the birth certificate, but left us when she was only a few weeks old. I know that it is possible to change her name if I have the father's permission, but what if her father moved to another state and I cannot get a hold of him...can I still proceed without it?

I do not see why you can't change the name. Are you interested in terminating his parental rights? Is he paying child support?
