my daughters father hasn't been apart of her life for at least a year. i wont let her around him because when he would finally get her, there would be signs of neglect when he sent her back.he took me to court for it and he never showed up. they ordered supervised visitation if he wanted to see her. he was in jail for burglary for 6 months and just got out.i fear he's going to try and walk back in her life. child support has been ordered and he hasn't paid. I'm hoping he'll stay away if i dont report him;and i think thats what hes hoping for to. I want my daughters last name changed, and i was told i needed his consent, but after i request it if he dont show up for court its automatically changed to mine. He has a bad habit for not showing up for court, so im kinda depending on that. i'd like to know the steps exactly to do that. And also i was told if he's out of her life more more than a year or doesnt pay support for over a year he can get his rights relenqueshed. I'd like to know how to go about doing that as well. i'd appreciate any help. Thank you.

To get the name change you need to contact the probate court. They should have the necessary forms that you need in order to complete the process.
To get his rights relinquished you need to file a motion with the court in order to start the process. Is he paying child support or visiting? Or is he doing nothing?
