need to File a motion to appear by Phone

I am representing myself, (not by choise), and I was told i need to file a ''motion to appear by phone'' and it has to be filed by Friday at the lastest.

how do i go about doing this?

If don't want to represent yourself, why haven't you contacted a lawyer?

A Motion to Appear by phone is simply a written document that appears with the heading of the case. In the body of the document just ask the court if you can attend by telephone. Then make sure they have a phone number where they can reach you at the specific time of day that the hearing is scheduled.
