Legal Question in Family Law in Ohio

My girlfriend of 9 months can not shake her ex boyfriend. She has changed addresses, changed phone numbers and blocked him on Facebook. We have 10 documented times where she has asked him to stop contacting her, just on Facebook alone, let alone texts. He just keeps finding new ways of trying to reach her. What can we do to stop this harassment?
