live in couple has child

We lived togather about 3 yeas in Ohio.I was asked to move out Nov 08 and my daughter was born March 09 and I was there.

Our child's last name was put on the birthcertificate as her mothers last marriage name without discussing this with me.I was asked to sign some papers at the hospitals and the lady told me if I signed them I would be responsible for all bills.I refused to sign after learning that my last name had not been given to the baby.She does let me visit the baby and goes out to dinner with me since but refuses to discuss our future with me about our daughter

child support or visitation in the future. I want to do the right thing for my daughter because I love her and still love her mother but I am very hurt that the mother would not give our daughter my last name instead she gave her last married name which is her legal name.

should I go to a lawyer?

If at the birth you signed an affidavit that you are the father, you may have to petition the court for a name change which would be granted if determined to be in the best interest of the child. It sounds as though you are experiencing custody/visitation issues as well. I suggest consulting with an attorney.
