modification of custody and visitation

My ex-husband has sent a letter stating he does not

wish to have visitation with his kids at this time. I would like to file for sole custody and modify visitation. how can i do this without a lawyer? i cannot seem to find legal forms to fill out for the court.

You need to file a motion to modify visitation, At the hearing you can state your reasons. Further, in many jurisdictions, you will need to file a parenting affidavit and an income and expense disclosure. These forms should be a vailable atthe clerk of courts in your county.
