Qualified Domestic Relations Order

I was divorced in December of 99 and my x-wife was supposed to receive money from my 401k. She never received the money. How do I go about getting this money out of my 401k. How do I get a Qualified Domestic Relations Order. Is this something I can take care of myself or do I need an attorney.

You need an attorney to prepare a QDRO as they are extremely detailed and complex.
