Legal Question in Family Law in Ohio

I received a letter from my ex - husband attorney for vistation modification my son is 16, the letter has the name I had when I was married to him 14 years ago , the attorney was informed that the name was wrong by my ex and he keeps sending in wrong name and said it does not matter, I am not comfortable opening any legal documents without my proper name on it. Can it be sent back
