Temporary custiody
My Daughter 17 Yrs old lives with her mother whom I share custody with, They live in Canada, Daughter is still USA citizen. Daughter wants to live with family member in Ohio. What sort of document do I need to obtain so she will be able to inroll in school and for the family member in Ohio to be able to represent her in a medical situation?

I would talk to the school district in Ohio and see what they want.
Each school district has different policies.
To take the child to a doctor, I used a medical power of attorney for my babysitter. Thankfully it was never used.
Again, see what a doctor in Ohio wants in order to treat her.
If it's an emergency (life and death) I believe that an emergency room will treat her before worrying about who is responsible.
Of course, most doctors and emergency rooms mainly want proof of medical insurance in order to treat someone.
I don't know Ohio law. In Texas a 17 yr. old is an adult for criminal purposes.
