Husband died/doctor lost his license for drinking
My husband passed away in October 2007. When I received his death certificate, it had something on it that didn't make sense. I found out a month later that our doctor had lost his license for drinking. Can I get a copy of my husband's medical records and where do I go to get these.

If an estate was opened, send a copy of the letters of appointment with a medical authorization to the doctor's office. If his office is closed contact the Ohio State Medical Board and determine where the medical records were transferred and placed in storage. If you did not open an estate a copy of the death certificate with a properly completed medical authoriztion should result in the same outcome.
