Legal Question in Health Care Law in Ohio
My mother signed a Living Will and durable power of attorney some years ago - she has moved since then, as have my brother and I, who are the executors - does the entire form need to be re-done, or can a notarized 'addendum' of sorts be added, to reflect our current addresses ?

The change of address should not be a problem. The power of attorney is connected to the person, not the street address. You might ask this question of a hospital or nursing home staff person on admission to see what the individual health care facility's stance is, but otherwise there should be no legal difficulty with using the same POA over time.
