How do I go about hiring and getting a visa for a nanny

The first issue you will face is that you must prove to the U.S. Department of Labor that you cannot find a qualified and willing nanny in the U.S. to take the position. Next, due to the inherent temporariness of the position, the visa for a nanny will be for a very short period of time (e.g., an H-2B visa is limited to one year, but can be extended).
