My company has been charging me a monhly fee for a family health insurance for two years, hoever the insurance states that only I am covered under the insurance. I have been paying additional cost for my wife, however she has not been insured for two years.

Is there an HR person at your company that you trust? If so, speak to her/him. Has your wife had any claims denied in those two years? If so, you may have some recourse to get those claims covered.
Another issue that may complicate the matter -- recently, many hospitalization carriers have been issuing separate policies for all the members of a group. So, for instance, I have a policy that covers my spouse, two of my kids as well as me. Each of us has our own separate policy and policy number. If a physician submits a bill for one of my kids under my policy number; the insurance company will reject it.
An additional complication may arise from the issue of whether or not you can add your wife at this point in time. Most policies limit the months that you can add a family member. For instance, I can only add family members (except under special circumstances) during October. If I miss that deadline, I have to wait until next October to add them.
