Re: termination of employment

If you are not exempt from the minimum wage/overtime requirements, the employer is required to provide the following information contained in your personnel file on request: 1.) name; 2.) address; 3.) occupation; 4.) pay rate; 5.) hours worked for each day worked and amounts paid. Additionally, employers are required to furnish medical reports relating to the employee making the request.
