Living trusts should contain information that is highly specific to your needs and desires regarding your property and family. A pre-printed online form will most likely not do what you desire. I would not recommend using a pre-printed living trust form.
If you are instead referring to a LIVING WILL, there is a standardized Ohio form that is widely accepted. There is nothing wrong with this form. In fact, many attorneys believe it is the best to use because it is so widely known and accepted. You do not need an attorney to sign off on this. You need either two witnesses (with specific qualifications detailed in the Living Will) or a notary to notarize the document.
