Disclaimer for TOD account

I am the TOD named on a deceased relative's investment portfolio. I want to disclaim the funds and turn them over to my sons, but my attorney will not supply that form to the investment firm. What can I do?

Contact the investment firm yourself. But also make sure to learn whether there will be undesired financial consequences, such as taxes, cost bases, etc.
