Your mother should consult an attorney; it may or may not be advisable for her to place you on title with a survivorship deed. There are several variables to consider, such as tax consequences, creditor protection issues, dower interests if either of you or mom are presently married. This is not an exhaustive list.
Also, there are other methods to have the property pass outside of probate, such as a Transfer of Death Deed, or a Trust.

I agree with Attorney Rifici's answer when making a change of ownership you need to consider all the consequences. A transfer on death deed may be your better option.

You get an attorney to draw one up for you. It is not an expensive procedure. Please feel free to contact me if you have further questions. Best regards, Edward DiCato
