Legal Question in Wills and Trusts in Ohio

Is there a separate form for "trust amendment and reinstatement"? If so, where do I get this form? I already have a living trust and I want to add/delete beneficiaries.

There is no standard form for a trust amendment or a restatement of trust. You would need to contact an attorney who specializes in this that can draft this document for you or you can try to create your own using some standard legal form you can find online or purchase legal software.
