You should first contacted the company or agency declaring the debt with a letter. This should be a letter of dispute. If you do not recieve a reasonable or factual explanation from the company or agency, you should contact all credit reporting agencies and the individual declaring the debt to have it removed.

To answer your question, the only reliable source for this debt documentation is the individual or company who claimed or created it. They must, by law, must provide this information and you have a right to dispute it. I hope this helps.
