15 year old son wants to live with dad

MY stepson wants to live with us and is moving in as of 8-9-02 and the child suport is garnished out of my husbands paycheck from work . Is there forms that we can down load from the web to fill out and get custady changed and to get the Judge to stop garnishment on his paychecks and if not how do we go about getting this done with out costing hunderds of $. Thanks for your question. I am assuming that the mother is agreeable to the change of custody. I do not know if lawguru has the necessary forms.
