The answer begins in your divorce decree and custody agreement. You may have shared decision making authority about you son's healthcare. If that's the case, consider documenting the medical need for the medication, including specific facts from people like his teachers which say he has a problem. If your ex then still refuses, then consider getting an attorney to get a legal action going to get court approval to do it against your ex's wishes.
