Legal Question in Medical Leave in Oklahoma

high fee for fmla paper

my pain managment dr charges 150 dollars to fill the form out. I have heard and even paid small fees for this . Is this legal?? i live in oklahoma

I don't know of a "law" prohibiting the doctor from charging for it, but I've never heard of a doctor doing so. I've always heard of the doctor completing the forms as part of the examination, although I have heard of some doctors charging a small administrative fee (e.g., $5 or $10).
