if a resident in a nursing home refuses treatment does he have to sign a form showing that he was informed that it could cause his death at the time in question?

It's not a criminal act to not have the refusal of medical treatment be in writing. Still, I would advise the nursing home to get the resident's refusal of medical treatment in writing; that should be logical of the home to do so.
