If a minor publishes a book about her stay at a mental hospital and mentions names and situations of other minors from the hospital, what are the legal ramifications that could occur. The hospital name is not mentioned, but her therapist worked at the hospital, and he has written an introduction to the book using his name.

You would be safest changing the names of all others involved and masking identifying details. Sounds like the book is about one person, who doesn't need to use real identities of other people to make the book work. Otherwise, you are looking for trouble identifying others in an institution, especially minors.
