Business License

I am a massage therapist living in oregon and I want to start practicing on the side. I have liability insurance, do I need a business license to operate if I am operating under my own name?

Business licenses are imposed by cities and some counties. It's not something that is a mystery, a simple phone call to city hall will tell you if there is one, and where you get the form to register your business (some cities let you do it online). There is an minimum annual fee, and some municipalities will charge you based on number of employees or revenues (which you will need to report on an annual basis).

In case you're in Portland, there is a business license fee.

Business liability insurance is for your protection, it has nothing to do with this "tax" (license) imposed by the government.

Taxes, licenses, and liability protection through insurance and legal entities are something a lawyer, an accountant, or another advisor can help you with.
