YOUR QUESTION:

How do I terminate my parental rights?

ANSWER:

First, you need to distinquish parental RIGHTS from parental RESPONSIBILITIES. The are two very different concepts.

As to parental rights (which is the subject of your question), the answer is rather simple. Your terminate your parental rights by simply not exercersing them. That means you take no action to exercise your right to have custody of your child, to visit with your child (if you do not have custody), maintain contact and communication with your child, etc. And that is the way you effectively terminate your parental rights. (Of course, you would still have parental responsibilities.)
