How does one file a Tort Claim against Oregon Health and Science University (OHSU) in Portland, Oregon

Go to ORS Chapter 30. I believe it's ORS 30.275, and the statutes following it. It's a little technical, but ought to be understandable.

To be really SAFE, though, it would pay to talk it over with a medical malpractice lawyer first, or see if you can get a lawyer to prepare it for you. NOTE that the TIME FRAME for filing your Tort Claim Notice can be as short as 180 DAYS from the date of the incident, so don't delay.
