Legal Question in Personal Injury in Oregon

In February I visited a local dentist for an exam and cleaning. It was determined that I needed 2 teeth filled. I made an appt. to have that done which resulted in a perforated front tooth. I had to return the following day and undergo more injections so the dentist could remove the filling in the front tooth and pack it with an anticeptic pack. As a result of that second visit and further injecitons I sustained a locked jaw. Although my dentist agreed to do the root canal ~ admitting he was responsible for the perforated tooth ~ he sent me to an Endodontist for the jaw issue. The Endodontist's diagnosis, regarding my locked jaw, was a condition called Trismus which is muscle or nerve damaged caused by multiple injections. I was sent to a cranial/facial specialist who said my jaw disk was displaced from the Trismus. He tried on three separate occassions to manipulate me back into place with no success. I ended up having to undergo a surgical procedure called an Arthrocentisis which was unsuccessful and now a more invasive surgery is scheduled. My husband and I met with this dentist to discuss the chain of events that had happened over the course of two and a half months and to ask him to take financial responsibilty as the injuries were done while under his care. He would not release his Insurance Company name to me but assured me he would get back to me by the weeks end. He didn't!!!

I filed a formal complaint with the Oregon Board of Dentistry and sent him a certified letter requesting his Insurance info. Not once did he ever call me or the referring Endodontist to check on my status and then he lied to me by not getting back to me after our May 11th meeting. What is my course of action against him?
