Article of Ammendment for company name change??

We want to switch our company name from its existing to a new one. What form do we have to file and where can we get it from?? Roughly what costs are involved, including time?

Article of amendment two copies of docketing statement, and consent to appropriation or use of similar name if applicable. Filing fee is $70. I would charge appromixately $125 to complete paperwork and file forms.

You would file Articles of Amendment. Click on forms at the Pa. Department of State, Corporations Bureau, website: http://www.dos.state.pa.us/corps/site/default.asp.
