Collections agencies

Is a collection agency required to send me statements (monthly or otherwise) and inform me of what account my payments are being applied to?

You should only pay a collection agency when you have a clear agreement with them as to credit for payments, the amount and duration of payments, and some assurance that it will discharge the debt that you owe to their client.
