House arrest??

How can I find a form to request my sentencing judge place me on house arrest? I cant find anything on this subject please help.

Your question is somewhat unclear.

If you have already been sentenced, then you file a motion for parole to house arrest.

If you are waiting for sentencing, you or your attorney can ask for house arrest instead of jail time at sentencing. However, this is not always appropriate. You should really ask your lawyer, or respond back with a lot more information.
