Can mother alter Doctors report?

My stepsons mother changed his diagnosis on a Physical form for camp from ADHD to ADD. The original Diagnosis was ADD but has since been changed to ADHD after an 8 day hospital observation. There is a distinct difference between the 2 disorders. Is that falsification of documents? We feel that she is deliberately misleading the camp as to what the child's true problems are, and she has done this before as we have many Dr. reports that show she has altered information according to what kind of meds she wants for the child. What can be done legally by us or by the Physician who wrote the report? She states that she is allowed to change it because that was his original diagnosis in Sept. 2002 even though Dr.'s have since changed it. Who do we take this paper to to show them that she is deliberately providing misleading information?

No. Nothing; the Doctor could send a corrective letter. the common pleas court if u are seeking to involuntarily terminate the Mother's rights.
