How do we get a POA revoked and replaced with someone else?

You revoke it by telling the present agent preferably by writing him I would also send a copy to any financial institutions that are aware of the POA. You can have another POA drawn which will indicate you are revoking all previous POA. I would suggest you contact an attorney to have this done.
