Legal Question in Family Law in Pennsylvania

I have a 8 year old and a 4 year son. The father of my children wants to change there last name does he need my permission to do a last name change. i pay childsupport to him everymonth and he keeps the kids from which im not allow to see them at all.

Is there a custody order? If there is then if legal custody is shared he probably needs your permission.
