Legal Question in Family Law in Pennsylvania

I have a biological child who is 20 years old. I have been married for 17 yrs. My husband has been her "Daddy" for all them years. She was an artificially inseminated child. My mother took her when she was 3 years old. We only recently got her back, she attempted suicide when she was 18. She wants to change her last name to our name and wants Dan as her legal Dad. How do we do this? We live in Pennsylvania.

As an adult she can change her name. Or that could be done as part of an adoption.
