Legal Question in Family Law in Pennsylvania
Boyfriend of 12 years added me to insurance they changed my last name to his and have me listed as spose. So if we separate do we need a divorce?

No. There is no such thing as a marriage by insurance company changing your name.
You can be added to the boyfriend's policy but the insurer probably made a mistake. PA no longer has common law marriage. Did you have a valid common law marriage? In that case, you would have to get a divorce, but not otherwise.
