Legal Question in Family Law in Pennsylvania
I have been with my boyfriend for 4 years, and he has a 5 year old son that lives with us. The child's mother signed away her parental rights when the child was 1 year old. We are not married but we do live together. We are just looking into our options for me to have rights over his son. I just need to be able to take him to the doctor and such. What are our options?

There seem to be three choices here: power of attorney, adoption, or legal guardianship. I'm skeptical about the first of those, but you can ask your pediatrician if he would accept such a document signed by your boyfriend to allow you to arrange medical care. In Allegheny County, both adoption and legal guardianship procedures go through Orphans Court. If you're not ready at this time to become the child's legal parent (assuming that the grounds exist for you to do so) I would recommend setting up a guardianship.
