change of minor last name

I have actually two questions i have been looking all over the net trying to find out how to go about changing my daughters last name so that me and her have the same last name she is my biological child and how to go about it for free and where in philadelphia to get the papers to file for minor name change.

Of course you can't do it for free. But if you go to your local courthouse and speak with the prothonotary (we don't understand why they can't just call it a clerk) they will be happy to help you fill out the paperwork. I can't direct you to the exactly correct paperwork without incurring the wrath of my liability insurance provider, but generally Philadelphia forms can be found here: http://courts.phila.gov/. You are looking for the form that specifies name change for a minor.

Depending on the age of the child you may need consent from the father or an order showing you have full custody. Otherwise it's a very easy process.
