name change

My 15yr.old daughter wants to change her last name to mine. Do I need an attorney or can I file myself and how do I go about doing this?

Some county prothonotaries give you forms and advice for changing a name. Check with that office. You are not required to have an attorney; but you will have some "red tape" to get around. When you retain an attorney it can be as much about letting someone else attend to the details as it is knowledge and advocacy. If your Prothonotary does not have forms to help you I suggest you first look at the file of someone else who went through a similar proceeding. Copy all the contents. Look up the law and the rules of procedure which relate to the name change. You should be able to put it together.
