changing a childs name

if a father signs away all parental rights to a child, does the mother have to have someone with her last name to adopt the child to give the child her birthright name that she goes by?

Just how does a father sign away all parental rights unless the child is to be adopted or the state is removing the child from a home due to abuse or neglect with the goal of having the child adopted.

The child's name is generally changed on the adoption decree.
