changing custody from parent to another parent at childs request

my husband was married before 2 girls 13,16, now . he has had them aince his devoice from 1st wife. we raised them . now at the ages they are they now want to live with mom . we are looking for a legal form to list the change that now mother assumes 100% of there wellbeing just like we did all these years . medical shes primary we sencondary which means anyoverand above it's her responsibality. we will pay her support on a weekly payment by check. when she goes back to work all her support will be returned to her along with support from us. she accepts responsibility for them and all intails. she gets to claim them in her taxes must stay a full year so none of us encurr any problems with the irs. schools doctors, etc... We want to protect ourself in every way but yet make exwife feel the same.what kind of form do we get.?

The form you need is a custody order. Which means Mom needs to file for custody and there needs to be language in the custody order that covers everything that you mentioned. I is also possible that dad could file the petition. for the order.

I must say its a refreshing change to see this happen with a total lack of hostility.

The sad fact is it best you not make this a do-it-yourself order. I would advise you to seek legal counsel. If for no other reason as handle the matter quickly and with a minimum of hassle.
