It's generally not a good idea on representing yourself in this kind of matter. If you proceed without a lawyer, you will be expected to know and comply with all of the PA rules of evidence and civil procedure and the local rules of evidence and civil procedure. I would highly suggest that you retain a lawyer. If you cannot afford one, then I would suggest that you contact your local legal aid or local bar association.

With that said, nobody can really tell you how to write this. An attorney does a thorough consultation to determine how to write this, so it is impossible to provide guidance here.

If you live in the central PA area and want to retain a lawyer, feel free to contact me.
