Legal Question in Family Law in Pennsylvania

I need to get custody transferred back to me of my daughter. She is with her godmother and she is willing to give her back to me. What is the proper paper called that needs to be filed and where can I get it?

Is there a custody order in place? If there is you and the god mother can file a stipulation that would modify the current via a petition with the court.

If there is not an order in place then I would need to know by what authority the godmother has custody.
