My ex is trying to take my daughter out of state permanently and is keeping my daughter from me. How can I stop her?

You need to file a petition with the Court of Common Pleas in your county asking that she not take the child out of state.

A Petition should be filed immediately, perhaps on an emergency basis un the county where the custody order was issued oi wherever the case is now.....
