errors in adoption papers concerning name and age

Upon adoption, my proper first name and my birthdate were recorded incorrectly. I am retired and I need a document for my wife/heirs to present explaining why birth certificate and adoption papers differ. My Life Insurance Co. has already questioned the birthdate. Rather than having each document certified everytime I need to establish who I am, I would like a ''one time'' document that can be copied and notorized and generally recognized when necessary. The birthdate that I celebrated for 65 years is incorrect and when I applied for SS I had to present certified documentation proving my correct birth date. My name on my adoption papers was a nickname rather than my legal name. I have been using my legal name as an adult, however, I have no proof of it except on my birth certificate which contains a different surname. The adoption papers use the nickname.

It would be advisable for you to contact an attorney in your area who practices before the bar of the Orphan's Court (the division of the Court of Common Pleas which deals with estate issues and adoptions) Depending upon the county, your attorney should be able to petition the court for relief in order to amend the record. The process would be dependent upon local procedures which are highly individual to each county.
