Father wants to relinquish parental rights

My daughter's biological father has agreed to sign over his parental rights. I cannot afford a lawyer and need to know how I could start this process without a lawyer. Should I go to the Prothonotary's Office? Also, I have a hearing coming up to have my daughter's last name changed, the biological father will be there to give his permission, could we possibly have him sign over his rights at that hearing in front of the judge?

Not so fast. First you need to go to the Register of Wills not the Prothonotary. Second, who is adopting the child?

Along with the Parental Rights his duty to provide support would also be terminated. So he can't just sign away his rights. If he could every parent subject to child support would go the same route. So unless the child is to be adopted he can't just sign over his rights.
