Need a form for ex-wife to sign over parental rights to me (paternal father)
What form do I need to have my ex-wife sign her parental rights over to me (father)? Then how would I process the document? Will I have a hearing, or do I file it at the courthouse, what would be next? I live in PA.

There is such a form however, its used in conjunction with a petition of adoption.
In short absent Children and Youth terminating a parents right due to abuse or neglect. The only other way parental rights are terminated is if the child is to be adopted.
