I am not clear on what you are talking about. There are no forms, although a father, if the parents are not married, can sign an acknowledgement of paternity, when the child is born. Are you releasing custody to someone else or are they releasing custody of the child to you? You and they will then need a custody agreement. Is the child in PA at this time? Is this a situation where the parent wants the child to live with you so the child can go to a better school? You might need a custody order from the court in that case. Either way (court order or agreement) the child's parents should contact an a family law attorney to have this done properly.
