What if my grandchild who is 12 years old wants to live with me instead of his mother and stepfather. They live in Nevada I live in Pennsylvania

If your grandchild lives in Nevada, odds are that any child custody dispute would have to be handled in Nevada. Pennsylvania usually will not touch a custody case unless it is the "home state" of the child, which is to say that Pennsylvania was the last place that the child resided for a full six-month period at the time the case is filed.
Fifty states might as well be fifty independent countries as far as most family law is concerned. Every state has its own way of handling things, and you will want to take a look at Nevada child custody law, particularly concerning the circumstances under which grandparents have "standing" to claim custody. Odds are, as a grandparent your biggest barrier will be your right to claim custody in the first place. In Pennsylvania, grandparents can claim primary physical custody of a child only in certain clearly-defined circumstances.
